    What thread size is this?

This is my jockey shifter arm. It's 3/8" and coarse threads but am I looking at X 12? A 3/8X16 coarse threaded knob would fit this fine right?

Sorry for the elementary question but had to ask.

I believe you are right as the X = number of threads and your shifter arm being a X 12 and the knob X 16 means you can screw the knob all the way down without pushing through the top of the knob you may need some loctite to stop it vibrating loose I would suggest screwing on without loctite first without forcing to make sure of your threads are compatible but I don't think it will be an issue
What will it matter in 100 years

The 12 and 16 are the number of threads per inch not the length of the thread. A 12 would be coarse and 16 fine. You can look at the threads and see the difference and they cannot be interchanged.

If you don't have a thread gauge just hold a ruler against the bolt and count the threads.

I do this all the time
messure 1/2" on the threads, count how many threads there is in that 1/2" and double it and that is the thread pitch
so if the major diameter is 3/8 inch and you count 8 threads in 1/2" of thread, then the thread is 3/8"-16
    got it?
oh yah and 3/8-16 is a corse thread and 3/8"-24 is a fine
